Depending on your health insurance you may need an authorization form from your primary care physician prior to your exam. It is important that you can contact your insurance carrier if you have any questions regarding your coverage. Alpha Advanced uses a private billing company called Per' Se Technologies who will submit all claims to help ensure your insurance is correctly processed. If you have any questions about your bill, please call 1-877-251-8787 (Monday - Friday, 10am - 3pm).
